It's very low angle on the foreground figures so we are looking up at them, so penis-head guy can't be standing on the floor. But even if he is on some kind of raised pedestal he's standing at something like an 80 degree angle to the other figures. The only way I can see this working is if the foreground figures are actually leaning over really far.
